Hello,
When I receive submissions, the order pickup date ad time are not showing, even though they are required fields. Please help, thank you!

The field is hidden in the form due to a misconfigured conditional logic. This is causing the problem in the form.
I have fixed this for you by deleting the conditional logic.

The field is blank for the two submissions in the form. You may consider asking the submitters to edit their submission to add the date.
Get the Submission ID:
https://www.jotform.com/help/549-Where-to-Find-the-Submission-ID
Next, append the ID in the following URL: http://www.jotform.com/edit/XXXXX where XXXX is the submission ID.
