I keep getting auto-logged out, have request password and my form lost

I am a new user. I've had a frustrating experience in that I seem to get auto-logged out. The system then does not remember my password (I have saved it and know I'm not getting it wrong myself), and I have to keep requesting password re-sets (which are then not remembered by the system again). My created forms seem to be lost and that I need to start all over again!

Do you know why this is happening? Is there also a way I can save the form to prevent losing it again?

Asking for an access code to conditionally show/hide fields within your form is okay. However, you should refrain from using "password" as your label to avoid being flagged by our phishing detector. Kindly use something similar like an "access code". Other than that, no other kinds of passwords or user credentials are allowed to be collected.
