I finished my form and it says I am locked out

    I tried to log in from my phone, was using the wrong password, I answered questions and am still locked out. Please let me know what I need to to do asap as I need my form

    This form https://www.jotform.com/90946505146157 was found on your account as suspicious of phishing activity because you're collecting social security and driver's license numbers.

    I need to collect things like social security number and driver's license number so I can run background checks on prospective tenants. So you're saying I cannot do this with jotform? After I spent two hours building my form. Great let me know but it looks like I'm going to have to use a different service.
