What type of form do I use?

Why did my supele for revert to one page or multiple questions? Was it because I added a check box to the contact page? 

Here's what I want- 

Page 1- introduction-plus questions, text etc... the survey

Page 2- a score page- hidden

Page 3 - responses to score ranges.

Page 4- Contact form - Submit

Page 5- Thank you.

Please note that the form layout will not be changed automatically. The form layout can be changed from the form settings.

If you want to have multiple fields displayed on each page, you may want to use the classic form layout by setting up page breaks for multi-page form.

The card form layout can only display one question in each page. Based on the requirement provided by you, you may consider using the classic form layout.
