Revision History: Recent form changes are not reflected on the logs.

    Hello JotForm, 

    We have been working on a form for some time and it has several conditions.  I just received an automated email from the form that I shouldn't have so I went on to the form to take a look to see if there was an error.  When I logged on all of the conditions we had created were no longer present... When I went onto the form itself, I see no revision history for the past two months and still no conditions. 

    Was Jotform updated and is there any reason why these conditions were deleted?

    I have just checked your form and the revision history, as far as I can see there is a last change where the conditions were removed, here is what I can see on my end:

    Hello Kevin, 

    As you can see in the screenshot below -  I have indeed edited the form quite a bit in the past two months. So have several of my co-workers. (despite what you displayed on the form screenshot)  But none of this information is there and all we see is the revision history you have included in the above screenshot. 

    Is there anyway you can dig deeper to see the actual revision history beyond what we can see on our side of things? As it, literally, disappeared today... along with about 10 hours of conditional work and 400 University of Washington employees that now have an non-functional form.  

    Thanks for any additional help you can provide!  A phone call would be much appreciated as well if that is an option!
