How can I get access to form results?

We have a been using the form located at: https://form.jotform.com/70466304772154 to let people apply for grants from our organization.  The person who was in charge of the account with this form is no longer with the organization, and we have been unable to edit the form, or retrieve any responses.  We do not know the account name, or email it was created under, or where the form responses are being sent.  What can I do to get ownership of this form?

Please try to contact the account owner. Request that the account e-mail address is replaced with yours. The owner can do this in the profile settings page.
