How to modify my form to make it look more professional?

I have been putting many hours on my online questionnaire but still not pleased with the look and feel of it.

I think it lacks a touch of professionalism.

I am not sure what you are specifically trying to achieve. Kindly explain what changes you are looking to do in your form.
