Login failed - AUTOSUSPENDED

Hello There,

First. Dentist has been auto suspended from our account.... We have followed guidelines. I was creating a section in our form for a client to provide a company description. 

Example:

"Home Page (main menu)

Please provide an intro paragraph about your company. Please upload Word file below:"

Then I was suspended.... 

Please help us figure out the problem - thank you

The form http://www.jotform.ca/form/91006796127257 was found on your account as suspicious of phishing activity because you're collecting user credentials.

Gathering this type of data violates our Terms of Use.

Asking for an access code to conditionally show/hide fields within your form is okay. However, you should refrain from using "password" as your label to avoid being flagged by our phishing detector. Kindly change it to something similar like an "access code". Other than that, no other kinds of passwords or user credentials are allowed to be collected.
