I am having issues with the mobile view version of my contact form

hello!! so i am having issues with the mobile view version of my contract. Instead of it being sized properly for phone viewing, the clients are having to scroll over to the right inorder to answer the rest of the questions. It even does it when I am viewing it on the mobile viewer in preview mode. 

can you help me make my clients life a bit easier?

Our apologies. The problem is related to the Single and Multiple Choices fields which we fixed a few minutes ago. I checked the form, and it is no longer scrolling. Please check again to your end and let us know if the issue persists.
