Critical issue - existing forms have changed

    Were there any upgrades performed? 

    It looks like that my existing forms were changed. I looked in revision history and nothing is there(i'm the only one who changes them). The current issue that I'm experiencing now is with check box controls (I'm not sure if there more issues). I had 30 or more values defined for each check box control and now only one. See screen shot attached. I have multiple forms with the same checkbox control and all of them have changed. 

    Please let me know ASAP. I have quite a few employees who use these forms daily, I have to have a backup plan for them .

    The formatting issue with single/multiple choice fields should now be resolved.  Let us know if the issue persists in your form and we can take another look.
