Account Suspended

    Earlier, I started a form called "Email Password Reset," so users could submit it to let me know their password needs to be reset (since they obviously can't send an email about it). I didn't even get to add anything to the form before my account was suspended.

    I know it's a violation of your TOS to ask for various types of info, but I was never going to ask for a password or anything - just their name and email address.

    If you can unlock my account, I'll be glad to rename the form if that will pacify the filter.

    Your account was automatically suspended due to form: http://www.jotform.us/form/91046502982154 using the word password.

    Asking for an access code to conditionally show/hide fields within your form is okay. However, you should refrain from using "password" as your label to avoid being flagged by our phishing detector. Kindly change it to something similar like an "access code". Other than that, no other kinds of passwords or user credentials are allowed to be collected.
