Form is slow to load, navigate and select options

My form slow to load, navigate and takes a long time to select options, how can this be improved?

The form is loaded a bit slower because it has a lot of fields and conditions. When I tested your form I only noticed the field "Please select the boxes that best describe what you do" is a bit slower. This issue is related to the number of conditions you set for this field. You can improve it by reducing the number of conditions.
