notification emails are sent to the wrong person

    The emails and conditions for the Incident/Accident Report Form (see the URL provided) have been properly set up, yet one of our managers receives other locations' reports. 

    Please review the screen shots (4) attached and fix the issue for us.

    I have tested  a cloned version of your form and it seems that the notification email is sent to the correct email address.

    May we know the other email notifications that are sent to the same email address?

    I have cleared your form cache, can you please test your form again and let us know if the issue still remains?
