Is there an option to pre-fill some fields in the form that is unique for each user?

If the goal is to send a prepopulated form that is different for each of your users, you can use URL parameters to pre-fill some fields in the form. For example:

🔘 Full name field prepopulated with Dr. John Doe Smith M.D.:

https://form.jotform.com/53533976064967?fullName[prefix]=Dr.&fullName[first]=John&fullName[middle]=Doe&fullName[last]=Smith&fullName[suffix]=M.D.

🔘 Full name field prepopulated with Ms. Jane Doe Smith:

https://form.jotform.com/53533976064967?fullName[prefix]=Ms.&fullName[first]=Jane&fullName[middle]=Doe&fullName[last]=Smith
