Forms are not working

Hi guys

I´m working on a form and having big trouble making it work. 
I need some logic in it which is not working and when i delete fields which i already have put logic into - they are still in the form when i try them live.

This is the form - when i preview and click on the first option a new filed is beeing revealed which i have deleted
