These forms were found on your account and flagged as fake services scam.

We understand this might not be the case with your form, yet we had to act according to the general rule to be in compliance with our Terms: Free accounts with non-corporate emails showing suspicious forms must be suspended immediately.

In order to reactivate your account, we will have to delete the offending forms.

Or, kindly provide us the following information:

    1. The URL where your forms are used.

    2. Your website or proof of affiliation to a legitimate institution.

We also need you to change your registered email to a corporate one to avoid getting flagged again by our phishing detector.

Send these proofs to reviewers@jotform.com with Thread #1807702 as your subject. We will let you know about the status of your account after a thorough review of your proofs.

    Give me a break!
I’m the President of the Utah County Chapter (#753) of the Experimental Aircraft Association. I am testing JotForms to determine if I want to pay for the service going forward. I was pretty impressed until my temporary account was suspended. Now I’m likely going to use a competitor unless my account is immediately re-activated.
You can see the event announcement that utilizes 2 of the JotForms here: https://www.753.eaachapter.org/events.htm The third JotForm won’t be used until July if at all.
The website shows proof of affiliation with a legitimate institution, namely, our local 501(c)(3) EAA Chapter. If the IRS believes we are a legitimate institution I can’t understand why you have a problem with us.
I’m extremely disappointed. Please immediately re-active my account.
    Thank you,
    Michael A. Duke
