I am pretty sure the calendar option was working for me before on time and date. I cannot select date on google/chrome.
the calendar does not show up on IE.

Remove the whole form source code that you have inserted, starting from
up to </html> (the first one of two)
Insert this Doctype definition at the beginning of your webpage's source, above everything else.<!DOCTYPE html PUBLIC "-//W3C//DTD XHTML 1.0 Transitional//EN" "http://www.w3.org/TR/xhtml1/DTD/xhtml1-transitional.dtd">
Insert proper embed codes. You can get embed codes by going to Setup & Embed > Embed Form > then choose your preferred method of embedding. In your case, I strongly suggest that you choose Source.
An important reminder:
Your form's second date field is messed up. I suggest that you delete it and insert a new one.
