    Hi,

Looking at your site's source code, I can tell that you just copied the whole source code of your form in this page http://www.jotform.com/10742408411. Doing so created redundant tags in your webpage which resulted to conflicts in the JavaScript code that is reponsible for the calendar.

    Solution:

Remove the whole form source code that you have inserted, starting from

    <!DOCTYPE HTML PUBLIC "-//W3C//DTD HTML 4.01//EN" "http://www.w3.org/TR/html4/strict.dtd">

up to </html> (the first one of two)

Insert this Doctype definition at the beginning of your webpage's source, above everything else.

     <!DOCTYPE html PUBLIC "-//W3C//DTD XHTML 1.0 Transitional//EN" "http://www.w3.org/TR/xhtml1/DTD/xhtml1-transitional.dtd">

Insert proper embed codes. You can get embed codes by going to Setup & Embed > Embed Form > then choose your preferred method of embedding. In your case, I strongly suggest that you choose Source.

    An important reminder:

Your form's second date field is messed up. I suggest that you delete it and insert a new one.

As I controlled it now, it was working fine on all versions of IE (8,9,10). If the problem persists please feel free to contact us again!

    Thanks.
