How can I embed a simple sign-up for on my Wordpress.com website?

I'm trying to create a simple email subscriber sign up form for my Wordpress website. I've tried several ways, but when I paste the code into my customizer it says "failed to embed". If I publish, it does show up on my page exactly where I want it, but it's not the form I created. Can you help?

It looks like you've embedded it on "Visual" tab. Please click on the "Text" tab and embed the code inside that tab.
