I am a teacher at a high school and we use jotform to have other teachers refer students to a structured study hall during lunch. At the moment we have 1 student per form submission. I have been asked to have multiple students added to the form so teachers can refer multiple students who are missing the same homework assignment. I created a form that had 8 student name fields but when it integrates into Google Spreadsheet the form is not easy to work with or remove students who have completed their work.
Does anyone know how I can create a form that will integrate with Google Spreadsheet without creating a ton of student name headers and cause confusion with working with the spreadsheet? I have attached a sample of what the spreadsheet looks like now. I would like the multi-student form to integrate the same way the single student form does. Is that possible?

Unfortunately, every field that you add to a form will create a header on the spreadsheet.
However, here is a form that may be a solution for you:
I have embedded your form on Free HTML fields on a separate form. I then deleted the submit button and added some collapse fields for easier navigation.
Because the embedded form is the same one but in different instances, you will only have one spreadsheet with one name column.
