Can I upload a consent form to use (in PDF or Word format) as is?

    Hi there,

    I work for a dental program with Neighborhood Health Center and we use paper consent forms to get parental permission for students we provide services for.


    We are interested in switching to all electronic forms and create online child medical consent forms. Can I upload the form we have already created and use that as is or do we need to create a new form using your templates?

    I've included a screenshot of our form that we use.

    May we know if the PDF document is a fillable PDF?

    I’ve attached a fillable version of the form. I’ve tried to upload it using the link below but I don’t think it’s working because I can’t view it. Please let me know if you are able to upload it or if there is something else I should try.

    To clarify, do you want to convert your fillable PDF to an online form or convert it to fillable PDF in jotform? Also, please provide us with a download link of your fillable PDF so we can test it on our end.
