Cannot preview the form and it crashes.

Under Javascript section, it's probably conflicting with one of your JS script. Can you please try replacing the sample image below with an image file instead of injecting an actual code?

I've cloned your form and tried removing that field and the form started to load. Please let us know if replacing it with image works for you.

Thank you so much for the quick reply.

That was the issue, form works perfectly now. I couldn't even get the dev tools window to open, to see what the issue was and didn't think about that code.
