Hello,
When the respondent received his filled form in PDF format in his email, the PDF is blank (as attached).
Grateful if you could look into this matter

I am sorry for the trouble caused to you. I did check your form's PDF document and it seems in first 4 pages you have added blank box elements. Please check the screenshot below:
I would suggest you to please delete the first 4 pages of PDF document and that will solve your problem. Alternatively, you can also delete the existing PDF document and add a new one. Please check the screenshot below on how to delete PDF document and add a new one in "PDF Editor":

Actually respondents fill it as an online form and not as PDF form. So how to proceed?

It is not PDF issue but the PDF document. The same PDF document is sent in attachment when the online web form is submitted.
The best way to solve this problem is to edit the PDF document of your form and delete the blank pages. Alternatively, you can also delete the "PDF Document" and add a new "PDF Document" (not PDF form).
