How to add information as text to a form?

Hi,

I need to add the following information to our form.  How do I do this?

By completing this application and submitting it, you are agreeing to:

·        Pay the full program fee in full before training begins

·        Attend ALL program sessions, for 3 hours* at the United Way of Greenville County

o   July 16th *

o   July 23rd 

o   July 30th

o   August 6th

o   August 13th

§  July 16th - Program Orientation and Board Basics from 12 – 5 pm.

·        Enter the Greenville Chamber of Commerce’s Leadership Pipeline with the intent to serve on a nonprofit board or committee within the next 12 months.

·        Use the training and acquired skills to expand my role as a community leader.

 

Application Requirements:

Complete the entire application, including all questions, volunteer history, references, supervisors’ signature, etc. as it will be used during the application process. Incomplete applications will not be considered Seek approval and signature from your supervisor or employer to participate. If you are self-employed, this does not apply All applicants will be interviewed. You should be prepared to answer interview style questions similar to the ones in this application Applicants will be notified by June 26th with a determination on participation status. If accepted to the program, please share any accommodations needed.

The Application Deadline is June 21st, 2019. If your application is received after the deadline, it will be considered for the next class.

If you need to add information as 'text' to form, you can use TEXT element and then to format text as you wish. Here is a screenshot:
