I do not have access to the user name and password on our account because the person who set it up is no longer on staff. please help! 619-818-6359

Do you still have access to the email address that was used in the registration of that account? If yes, then recovering your username and password is as easy as clicking on Forgot Password button on the Sign In area on the lower part of Jotform's home page.
You will then receive your username along with instructions on how to change/reset your password.

We tried that, with all the emails that could have been used, but apparently none of our work emails were used and the person who set up the account is no longer with us. Hence our conundrum... We need to be able to access that account to change some of the forms on our site without having to reinvent the wheel...

Your username is jadeorchard and your registered email address is email@example.com. You can use the Forget Password option as described by NeilVicente above to recover your password. Or we can reset the password for you if you no longer have access to this email account. Just tell us where to send the password. For security purposes, we cannot post your password here.
