It might be a problem with your browser, as your account looks ok in our side. Can you please use Google Chrome to login ?
If you forgot your password, you may reset your password, here's how:
1. On the landing page (www.jotform.com) click LOGIN found on the upper-right part.
2. Click Forgot Password?
3. You will then be prompted to enter your email, so enter your email (the one you used in creating your JotForm account)
4. Click Send Reset Instructions.
