Why do my forms keep getting disabled?

    I have made 2 forms two different times and I keep having to remake them. Can you guys retrieve them?

    This form http://www.jotform.us/form/91505118783156 was found on your account and flagged as copyright logo scam.

    We understand this might not be the case with your form, yet we had to act according to the general rule to be in compliance with our TermsFree accounts with non-corporate emails showing suspicious forms must be suspended immediately

    In order to reactivate your account, kindly provide us the following information:

    1. The URL where your forms are used.

    2. Your website or proof of affiliation to a legitimate institution.

    3. Share with us a document that will prove that you're authorized to use the said logos.

    We also need you to change your registered email to a corporate one to avoid getting flagged again by our phishing detector.

    Send these proofs to reviewers@jotform.com with Thread #1849471 as your subject. We will let you know about the status of your account after a thorough review of your proofs.

    Due to our PCI certification, we are required to deny the collection of credit card details on regular forms.

    However, we have other PCI-compliant ways to do this. All you need to do is choose any of the payment processors available in our platform in order to integrate in your forms. You still won't ` be able to collect credit card details directly though.

    If you only want to collect your customer's payment details and charge them at a later date, you can take advantage of a feature called "Payment Authorization". To learn more, kindly follow this guide on How to Enable Payment Authorization.

    Or, we can just remove the logos and the credit card information and lift the suspension on your account.
