More than one multiple choice selection

    I was wondering if you have a option that allows me to include more than one yes or no option to select without having to actually add multiple individual choice 

    example:

    Please specify does the client have any other specific support needs:

    Drugs/alcohol            Yes or No

    Immigration                Yes or No                  

    Mental health                 Yes or No
