account suspended, please fix ASAP

    Hi, 

    Don't know what happened, but I returned to my jot account after I found out your were coming out with mobile apps. Was looking at using it in a browser until then, checking appearance on both a tablet and a PC around the same time. Suddenly, I get a message suspending my account for suspected phishing. 

    Please give me access before I lose interest in testing your software again for our use.

    This form http://www.jotform.us/form/53008530541142 was found on your account as suspicious of phishing activity because you're collecting social security and ID numbers.

    Online Form Builders, including Jotform, are sometimes being used for identity theft and we go to great lengths to prevent this. That is why collecting these types of data is considered as phishing activity, as stated in our Terms of Use.

    If your purpose is not to collect these data from the general public, but from your own customers, then you need to comply with two things:

    1. Add the Terms & Conditions Widget (or the Short Scrollable Terms Widget) for your users to read and agree.

    2. Add a Signature Widget so your users can sign and agree to provide such sensitive information.



    We’ve removed the offending field on your form and lifted the suspension on your account. Please refrain from making new forms that is against our terms to avoid this kind of inconvenience.

    Well, please tell me what to do because I recognize this form as one I got from your collection of templates, looking at perhaps modifying and using. But definitely hadn't added the SSN number field, made the form live, or distributed a link to anyone. 

    Please advise.

    As checked, account suspension has been lifted already by my colleague. You may now continue using the form.

    Moving forward, please refrain from using fields that would collect sensitive user's information such as sensitive credit card information, social security numbers, user login credentials, or other sensitive personal information. All these activities are considered as Phishing and any account along with the violating forms will be suspended immediately.

    Do you think all your other customers are aware that they can get their account shut down just by opening up one of your provided templates without modifying it or making it available to anyone?

    In regards to that specific form, since you mentioned it that it was taking from our templates gallery, we apologize for the inconveniences this may have caused you, we check for templates daily in order to remove the ones with fields infringing our Terms of Use, however if you ever find forms collecting sensitive information, we appreciate that you would inform us and we will take it down.

    The form previously mentioned, seems that it has been removed.
