Hi,
I want to create a 'healthcheck' form where users answer a series of questions which generates a recommendation for them based off the back of their answers.
Am I able to do this with this platform?
Cheers,
F.

Well, you can achieve this using conditions. You can add in your recommendation in a "Text" field in the form and keep it hidden. Based on the question answered by the user, you can show it.
This previous thread somewhat captures what you want to achieve: https://www.jotform.com/answers/1162867. Do review and try it out in your form.

Also, you can check out our Medical form templates, which come under HIPAA.

Hi Gerard,
Yes I’m comfortable with logic and hiding/showing responses. What I can’t seem to work out is how to email the respondent the resulting conditional text.
As an example:
Question 1.How much do you smoke?
Answer 1. 20 a day
Answer 2. 10 a day
Answer 3. 0 a day
Conditional Text Response 1. This is very bad
Conditional Text Response 2. This is very bad
Conditional Text Response 2. This is good
If Jane Doe responds to Question 1 with Answer 2, the form would reveal Conditional Text Response 2. This is very bad
On completion of the form, Jane Doe would receive an automated email saying:
Hi Jane,
You smoke [Answer 2] this is [Conditional Text Response 2]. Get in touch for help quitting.
Cheers,
Freya

You may use the update/calculate field condition and add assign your conditional text to a field.
Sample Condition:
Then in your notification email, you can edit your email content.
Here is my sample form you can test:https://form.jotform.com/91644023703955
You may clone my form so that you can check the condition and field.

Please give it a try and let us know if you have further questions.
