Where do I find my account?

I can't remember if I set up a user name or not. I still can not access any of my registrations for my summer camp or find the original set up for my form. I am needing to see who is registered, but I am not getting an email for my password. 

It seems the form is located under a guest account. If you wish to get access to the form, you may try using the same browser that was previously used when the form was created since browser cookies are used to identify the account who created the forms.
