We have a form that we would like be printed with header and footer that will replace the generic details.
Basically our goal is to print the form so it will printed just like a stationery.
Can you help me with this?

Are you referring to printing your submissions result? If it is, please check this thread, it discusses how you can show the header text so it can be included in the print output.
If you were meaning to use the print button in the form itself, you can just add a header field or free text(html) field on top and bottom of your form to have a header and footer included in the print output.
