We do not have a password protect feature form. Please keep in mind that your account will be suspended immediately if our system finds out that you have a password field or asking sensitive information to your form.

I created a workaround that is using the Show and Hide condition. You can test and check the demo form on this link: https://form.jotform.com/91675817509974

If the data in the Access Code is correct, it will show all the form fields. For more information about Show and Hide condition, please open our official guide:https://www.jotform.com/help/316-How-to-Show-or-Hide-Fields-Base-on-User-s-Answer

Will that works for you? If yes, you can also clone the demo form by following this guide: https://www.jotform.com/help/42-How-to-Clone-an-Existing-Form-from-a-URL
