When I clone it the form loses all of the formatting.

    I want to clone a form that was made from a template.  When I clone it the form loses all of the formatting.

    The template from which you clone form, is it a JotForm? Please share the form template URL and we will take a look.

    The image that you shared is a classic form (all question in a single page), but the form link that you shared is a card form (single question per page). You may have selected the wrong layout while importing. Probably, that must be the reason why the format was changed. I am not sure which format you wish to have, but you can always change the layout. 

    Please go to Settings > Form Settings > Show More Options > Form Layout and select the layout that you prefer.
