Account suspended

I would like to know why my account has been suspended.  My username is dauman_remora.

Upon review of your account it was found your form to be collecting IDs on upload buttons and also Banking sensitive information. This is not allowed per our terms of use unless you add the two mandatory widgets for collection.

1. Add Terms of Use widget (for your users to read and agree). This will surely make your forms not to be considered as guilty of phishing activities

 2. Add a Signature widget where your users sign and agree to provide such sensitive information.

We needed to remove the fields for compliance and to able to reactivate your account.

Please add the widgets at your earliest convenience.
