My forms were blocked

Two days ago I went to login and if said my forms were suspended due to phishing and requested I fill out a form with all my info. I still haven’t heard anything back after two days.

As I stated in the forms; I am a licensed and insured professional in the State of Virginia as a Permanent Cosmetic Studio & Tattoo Artist. I have a brick & mortar studio and can provide any documentation necessary.

I worked very hard to get this forms just right. Please let me know if they are going to release my account from suspension.
