Not Receiving Email Sign Ups

Hi there,

We are running a form and are not receiving emails when people sign up. On my end, the email info is all correct. Any idea why we wouldn't be getting the emails? We used to get them but now they aren't coming through. This is for the form 2nd Annual Chicago Golf Tournmanet.
