How to show multiple fields at once based on a selection.

    Hello! :)

    Hope I am going to explain this correctly. I am wanting to see if there is a way that when I put a condition to the "Intros" that if 3 is entered, then 3 of the "Name" & "closed or missed radials" are shown. (have done this for up to 10 already) but then is there a way to ensure that either the Membership & Notes or solely Notes field(s) show for each individual "closed or missed" choice?

    For example:

    2 intros are entered

    1. Mike closed is chosen so then the membership and notes fields show

    2. Stacey missed is chosen so then only the notes field shows up

    Want to have this possibility for the up to ten that could be entered. Is this possible? Or does it even make sense? lol

    What should happen with each radial choice...

    -Closed-->show Membership & Notes fields

    -Missed-->show Notes field

    Thank you so much!

    Thank you for contacting us, I understand that creating conditions could be confusing, but we will be glad to help you out with this.

    If I understood correctly, you would like to show a "Notes" field if the option called "Missed" is selected:

    And if "Closed" is selected, then you would like to show the "Notes" and "Membership/Packages" fields:

    This is possible, but first I would suggest you to put a divider on each section:

    And then update them in the conditions:

    Finally, for each Single Choice field that has the options called "Closed" and "Missed", create conditions like the following:

    Here is the example for the second set:

    I cloned the forms, and did this for the first two sets of fields, result: https://form.jotform.com/91748018316964

    So, if I do this with each radial choice 1- 10, then it will automatically update for the number entered into the Sales area?

    Just want to make sure I got it right. Thanks

    I am not sure what I am doing wrong but when I do "1" in this section it will show the membership (under a divider-not desired) and the Notes etc as desired for the outcome. But then when I enter 2 in the Intro section, it wont work properly. it throws the membership down below member #2. Then when I enter "3" or beyond. Doesn't even show/hide anything.

    Sorry, to be a pain, but not sure what is happening. Thank you

    Don't worry, setting this up could be confusing and time-consuming since the Single Choice fields are not labeled. I have taken the time to make this work in a clone version of your form: https://form.jotform.com/91748018316964

    Result:

    You may clone it in your account, and use it instead: https://www.jotform.com/help/42-How-to-Clone-an-Existing-Form-from-a-URL

    This is awesome! So sorry to bug but, when I went to enter "0" in this field all the "notes" and "membership/packages" showed up when none should, so went in and created a condition that if it said 0 to hide all of those. Yet when I put in the condition, go back and test "0" it now shows 10 lines of all the notes/membership-then when its blank it showed all the information for all 10 lines. So went back to delete the condition. Just need a way that if it says "0" like with the one previous of "fresh leads" nothing appears. How can I accomplish this?

    Thanks
