When I set up an email notification for a form, then run the test option, it looks fine in the actual email. When I use the form in my website, the notification email contains all html code. Help!

I can't seem to replicate your issue. The emails I received upon filling out a clone of your form do not show html codes - all I see is how exactly it looks while running a Test Email.
An advise I could give you is to delete your existing email notification and create a new one. You can do so by going to Setup & Embed > Email Alerts > Notification, click on Delete E-mail > click OK
Click on Email Alerts toolbar again, then choose Add New Email
