My form is not working

May we know if you were having the same issue when submitting the form from its direct link or from the link (custom URL) you shared in your post, please? Or, is it only happening when the form is submitted from the website where it is embedded?

I have submitted a clone version of your form and I was not able to reproduce the issue you are having.

May we ask you to share with us the exact link to your website where your form is embedded, please? Also, I noticed that in your screenshot you encountered the issue while submitting the embedded form on a mobile device with a Google Chrome app. Are you getting the same result when you submit it on a laptop/desktop computer?

With your permission, we would like to make test submissions to your form directly from the website where it is embedded.
