Phishing attempt using JotForm

Some of my users are reporting receiving a phishing email that wants them to submit their username and passwords using a JotForm: https://form.jotform.co/91835567414868

I'm fairly certain the use of JotForm for malicious intent violates your terms of service.  Can you please take whatever administrative action necessary to remove the customer using your service for these matters.
