Create new form from fillable PDF

We have been using a complicated fillable PDF form for a scholarship application. I've tried uploading/converting it but it did not convert successfully. Rather than try to tweak the result, it might make more sense to start from scratch and design the form correctly from the start. See attached form--this is what we want to replicate using JotForm. The users (applicants) have to complete this form AND upload documents. The results have to be available for review online and as PDFs.

The preview link is below; you can see how the PDF changed when it was converted into a form. We'd love some expert advice on how to make this application easier for users and our reviewers. 

I'm sorry but we didn't receive your PDF. Either way, if the PDF is too complicated then I'm afraid that it would be better to re-create it in the form builder (as you noted as well).
