I currently have 2 jotform accounts. One paid one free
On the free one i set up an online form that i wanted to integrate into Salesforce. But it didn't work.
I deleted the integration, reset the browser, logged out and then tried to set up the integration again. It still wasn't working. Then i tried to make a new form, and that one did integrate with salesforce. then i cloned the original form, and tried to integrate it with SF, but it did not work.
Then I imported the form into my paid account and here, the integration is working right away.
This is quite frustrating, as I am supposed to set this up for a client. I would appreciate immediate assistance in this matter.
I added more fields to sync to SF, and now the form stopped syncing to SF again.
Is there a limit on how many fields you can sync across?
The integration feature with Salesforce will work for Enterprise and Unlimited edition only. If you are not able to receive new records with even just the available fields for mapping, then most likely, your Salesforce account is incompatible.
The limitation of mapped fields from JotForm to SF is on the type of field.
Currently, the allowed field is for textbox input type of field only.
Other fields like dropdown box and radio option (which I see you have on your form) are not yet possible.

This is very unfortunate. Having a limitiation on the type of field makes jotform pretty useless in regards to SF integration. My client has the Enterprise edition.
I don't understand why the integration is that difficult.
Zapier can integrate jotform to Sf, and formassembly can integrate their forms to SF.
and both of them offer the option to create opportunities, tasks and so forth as well. So the API is there.
I really like Jotform, but not being able to connect to SF even though you clearly promote it, is very disappointing
I hope you can get on top of this sooner rather than later.
I also agree with you on the enhancement of the allowed field types.
I actually created a feature request ticket which I had submitted to our dev team. We will just have to wait for approval of the request.
We will inform you at once when the feature is available.
Any progress on this as a feature request? I have several checkboxes that I'd love to pass from JotForm to Salesforce. (Side note: Just discovered JotForm and am super excited about the potential!)
