I am a trained weather spotter for the national weather service. I want to be able to have the same standard form visible when I must do a weather report as a spotter. I want to be able to select my location (normally a road intersection), the start and end times of an event (i.e., a tornado strike, or merely the start or end of a squall or severe storm). Each message I send MUST include my own spotter ID number and my name. These reports must be sent to a specific NWS email address in Washington, DC, since I live in the area.
The NWS wants to receive a standard email, not a form and not a word or excel document. Will your forms builder allow me to do this?

Every form has it's notification email by default. In order to customize it according to your specifications like change the email format to Text or HTML (default format), include paragraphs, tables or images you like, you have to open your Email Notification by clicking Setup & Embed > Email Alerts > open Notification
Also, to find out what email address used for submissions just simply click "Reply-To and Recipient Settings" button as seen on the screen.
