Do you mean to say that after cloning the form you modified it but still the submissions are coming as old format?
Please share the form URL which you have used to clone.

We apologize for the inconvenience that may have caused. In order to fixed that, you need to re-create new email alert/notification because once you made some modification or update your form, it won't automatically updated your email alert/notification. You need to manually update that by either re-creating your email alert or fill in the new fields of your form in to your email alert/notification.
