How can I add a calendar (date) option as a sub category after my clients have selected the period of time of their membership?

    Hi,

    As you will see in my Summer camp form, at the very buttom there is an option "chose package" where my clients can select a period of time, however I'd like to include a subcategorie (calendar) where they are able to select the exact dates.

    I've just added "Date Time" field into your form then create a condition where if choose a package field is Filled then Show Date Time Field.
