Form not generating submissions

Got two submissions but other parties have shared they have RSVP however there is not record of it. I tried it on my end and also did not submit my RSVp.

The form is currently showing 5 received submissions (including one test submission from me). So far I was not able to reproduce the issue. I have tested a cloned version of your form, and also your form, in both cases the data was submitted properly.

Please kindly test the form again.
