Why is my account suspended?

    Hello,

    My questions is regains the account email - paul@bournemouth7s.com, username - Ashurst.

    Please help I'm going around in cirlces here with your website as I can't login due to my account being suspended? despite knowing the correct credentials. 

    I'm also having trouble sending this to you because is keep trying to get me to login which i can't because you've suspended this account.

    Still unable to login despite changing and using correct credentials.

    Hello.

    I briefly browsed through your account and found what caused your suspension. It seems that in your form named ''Casual Labour Invoice'', you have a field which asks for sensitive banking information. This creates suspicion for ''Phishing''. Any sensitive info used in forms is strictly against our ''Terms of Usage'' at JotForm.

    Here is the part which your form violates: 

    Phishing. You agree that JotForm, Inc. may terminate your JotForm Service immediately if a form's purpose is found to be deceptively obtaining, for example: sensitive credit card information, social security numbers, user login credentials, or other sensitive personal information. All these activities are considered as Phishing and any account along with the violating forms will be suspended immediately.

    Here is the part of your form which asks for the info:

    Since you can't access your account anymore, we can delete those fields for you and reactivate your account. In order to do so, we will need your confirmation.

    Hi Miles,
    Many thanks for coming back to me and apologies for going against your
    terms of use I was unaware. Please could you delete these fields and then
    reactivate my account?

    I have successfully deleted the suspicious fields in your form and re-activated your account. Please note that if you put fields that are against the Terms of Use, the account will be suspended again.
