Help creating a question/answer database

Hello, 

I want to create a question database for a biology course and am struggling to find a template. Do you have something available that would work so that I can create multiple-choice questions, add figures, images, etc. and apply specific tags (maybe by unit, chapter, concept, etc.?)

You can use one of our questionnaire templates to achieve what you need. 

To add images and figures you can use image field element available in the form builder.
