How to let the users submit the form twice?

I made a form for youth football registration. Some parents have more than one child playing and it will only let them submit one form. How can we change this so we can have forms on all players in one house?

Please go to Settings > Form Settings > Show More Options and set the Unique Submission field to No Check so the user can perform multiple submissions.
