You can download the full source code of the form and then add it to
    your Multiply Account.

Multiply does not allow JavaScript so you will need to
remove any javascript code from it befor posting it to your multiply page.

Open your form on the Form Builder:

    1. Open "Setup & Embed" tab on toolbar,
    2. Click on "Embed Form",
    3. From the options choose "Source",
    4. Copy the code displayed in Source Code,
    5. Before posting it to your page remove the JavaScript codes from it

    Hope this helps

    Hi Rowena,
I know the code below is a bit messy but Multiply is pretty strict when it comes to adding CSS or Javascript codes into their webpage editor.
Please copy the code below and paste it in a new Multiply blog post. Don't forget to check the Edit HTML box on the upper right corner of the editor box.
    <form class="jotform-form" action="http://www.jotform.com/submit.php" method="post" enctype="multipart/form-data" name="form_10902051230" id="10902051230" accept-charset="utf-8"><div style="text-align: left;">&nbsp;&nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp;</div><h1 id="header_3" class="form-header" style="text-align: left;">&nbsp;&nbsp; &nbsp; &nbsp; &nbsp; &nbsp;Rowena's Gems &amp;amp; Jewelry&nbsp;&nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp;</h1><div style="text-align: left;">&nbsp;&nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp;</div>
    <input type="hidden" name="formID" value="10902051230" />
    <h2 id="header_1" class="form-header" style="text-align: left;">Inquiry / Order Form&nbsp;&nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp;</h2><div style="text-align: left;"><label class="form-sub-label" for="first_5" id="sublabel_first"><br></label></div><div style="text-align: left;"><label class="form-sub-label" for="first_5" id="sublabel_first">First Name </label><span class="form-sub-label-container"><input class="form-textbox validate[required]" type="text" size="15" name="q5_fullName5[last]" id="last_5">&nbsp;&nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp;</span><span class="form-sub-label-container"><label class="form-sub-label" for="last_5" id="sublabel_last">Last Name&nbsp;</label></span><label class="form-sub-label" for="first_5" id="sublabel_first">&nbsp;</label><span class="form-sub-label-container"><input class="form-textbox validate[required]" type="text" size="15" name="q5_fullName5[last]" id="last_5">&nbsp;</span>&nbsp;&nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp;<label class="form-label-left" id="label_4" for="input_4">&nbsp;&nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp;</label></div><div style="text-align: left;"><label class="form-label-left" id="label_4" for="input_4">E-mail<span class="form-required">*</span>&nbsp;&nbsp; </label>&nbsp;&nbsp;&nbsp;<input type="email" class="form-textbox validate[required, Email]" id="input_4" name="q4_email4" size="30">&nbsp;&nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp;<label class="form-label-left" id="label_7" for="input_7">&nbsp;</label></div><div style="text-align: left;"><label class="form-label-left" id="label_7" for="input_7"><br></label></div><div style="text-align: left;"><label class="form-label-left" id="label_7" for="input_7">Item code or picture's file name </label>&nbsp;&nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp;</div><textarea id="input_7" class="form-textarea" name="q7_itemCode7" cols="40" rows="6" style="text-align: left;"></textarea><div style="text-align: left;"><br></div><div style="text-align: left;"><label class="form-label-left" id="label_8" for="input_8">Details of your inquiry or order </label>&nbsp;&nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp;</div><textarea id="input_8" class="form-textarea" name="q8_detailsOf" cols="40" rows="6" style="text-align: left;"></textarea><div style="text-align: left;">&nbsp;&nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp;</div><div class="form-header-group"><div style="text-align: left;">&nbsp;&nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp;</div><h3 id="header_9" class="form-header" style="text-align: left;">For Custom-Made Jewelry Orders&nbsp;&nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp;</h3><div style="text-align: left;"><label class="form-label-left" id="label_10" for="input_10">&nbsp;Upload Image(s) </label>&nbsp;&nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp;<input class="form-upload-multiple" type="file" id="input_10" name="q10_uploadImages10[]" multiple="multiple" file-accept="pdf, jpg, jpeg, png, gif" file-maxsize="10240">&nbsp;&nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp;</div><div style="text-align: left;"><br></div><div style="text-align: left;"><button id="input_2" type="submit" class="form-submit-button">&nbsp;&nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp;Submit Form&nbsp;&nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp;</button>&nbsp;&nbsp; &nbsp;&nbsp;<button id="input_reset_2" type="reset" class="form-submit-reset">&nbsp;&nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp;Clear Form&nbsp;&nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp;</button>&nbsp;&nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp;&nbsp;</div><input type="hidden" id="simple_spc" name="simple_spc" value="10902051230"></div></form>
Feel free to edit the alignment and text colors/style. Note that some of the eye candy visual effects that were made through CSS will be stripped off, but the basic form functionality of receiving your customers' feedback and uploaded picture will still work here.
Please let us know if you have any other questions or concerns. Thank you for using JotForm!

it really looks ugly when I tried to put it in my blog. I guess I'll try to redirect it outside of multiply then.

Other form builders are nice but I really wanted the file upload option

    thanks for trying to help

    Hi Rowena,

Our pleasure to help. Indeed it's a frustration for Multiply users because of the restriction on CSS and Javascripting in their html free form. Even Iframes don't work. URL redirect may be your best bet if you want the JotForm layout and appearance to be intact. If you need further assistance, please let us know.

Good luck and thank you for using JotForm!

I'm not quite familiar with multiply but you should post them on your blog content directly. As mentioned by my colleague's and base on my further research, multiply strips any script codes or even iframes so you have to use form HTML codes only if it is still possible.
